The Integrated Transfer of Care (ITOC) is looking for an experienced Clinical Therapy Lead. The role is open to physiotherapists or occupational therapists. The Clinical Therapy Lead will work in conjunction with the Clinical Nurse Lead and Team Leader to support the therapy aspect of the ITOC team.
Experience in community beds and discharge to assess/Recovery Hub services, complex discharge and knowledge of National Supported Discharge pathways is essential.
ITOC Model
- Reduce the time people spend in hospital when they no longer need acute care, preventing hospital‑acquired infections and deconditioning.
- Assess people in a more appropriate environment than the hospital, giving a more accurate indication of their strengths and needs.
- Provide multidisciplinary reablement and rehabilitation plans, and if necessary short‑term care and support, to help people regain independence and prevent or reduce need for long‑term care.
- Enable the urgent care system to prioritise acute hospital care for those who need it.
