Responsibilities

An exciting opportunity has become available within the North side of Kirklees for an experienced, motivated and compassionate Registered Nurse to join our Community Nursing Network as a Band 6 Senior Community Nurse.

This is an excellent opportunity to take on a leadership role, supporting our teams to deliver exceptional care across North Kirklees and Greater Huddersfield.

As a Band 6 Senior Community Nurse, you will combine clinical expertise with leadership skills to ensure safe, effective and person‑centred care. You will act as a role model, mentor and decision‑maker, supporting colleagues, driving improvements in patient outcomes and providing operational leadership in the absence of the Team Leader.

Key Responsibilities

  • Lead and coordinate care for patients with complex needs, ensuring high standards of clinical practice.
  • Provide guidance, supervision and mentorship to Band 5 nurses and wider team members.
  • Delegate tasks effectively to ensure efficient use of resources and timely delivery of care.
  • Support service development initiatives and contribute to quality improvement projects.
  • Act as a point of escalation for clinical decision‑making and safeguarding concerns.
  • Promote self‑care and independence for patients living with long‑term conditions.
  • Collaborate with multidisciplinary teams to prevent unnecessary hospital admissions.
  • Provide operational and clinical leadership in the absence of the Team Leader, supporting the coordination of staffing, workload and service delivery.
  • Act as a positive role model, helping to create a supportive team culture focused on quality, learning and continuous improvement.

What We’re Looking For

  • A Registered Nurse with substantial community nursing experience and a passion for leadership.
  • Strong clinical knowledge with the ability to manage complex caseloads.
  • Excellent communication, problem‑solving and decision‑making skills.
  • Confidence in delegation, coordination and team leadership.
  • A proactive approach to service improvement and innovation.

Working Pattern

This is a full‑time position of 37.5 hours per week. Our service operates between 8am and 9pm, with core hours of 8am to 5pm. Successful candidates will be expected to work a minimum of one weekend in four and undertake two late shifts (1pm to 9pm) within a four‑week period.

This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.
