Job Role: Band 5 Community Staff Nurse – Lower Limb Wound Care Team, 37.5 hours per week

Responsibilities:

  • Delivering high-quality nursing care to patients in clinics and home environments.
  • Undertaking holistic assessments of patients with lower limb wounds.
  • Using Doppler ultrasound techniques and clinical interpretation skills to support diagnosis and care planning.
  • Contributing to Complex Wound and Leg Ulcer Clinics across the service.
  • Coordinating care in collaboration with Community Nursing Network teams.
  • Supporting patient self-care, long-term condition management, and health promotion initiatives.
  • Contributing to innovative projects that enhance patient outcomes and service delivery.

Essential Requirements:

  • A full driving licence and access to a vehicle, as the role involves travel across Kirklees to attend clinics and visit patients at home.
