The predicted salary is between 38682 - 46580 £ per year.
This role offers a unique rotational opportunity across three dynamic service areas: Urgent Community Response (UCR), Integrated Transfer of Care (ITOC), and Intermediate Care (IMC). You will spend approximately one year in each setting, gaining broad and valuable experience across the full patient pathway, from rapid response in the community, safe and effective hospital discharge, to supporting recovery and independence in intermediate care.

Our Urgent Community Response team, part of Locala’s Integrated Community Services, is comprised of Nurses, Occupational Therapists, Physiotherapists, Advanced Clinical Practitioners, Assistant Practitioners and Healthcare Assistants. An opportunity has arisen for a motivated Occupational Therapist to join our team. The service covers Community settings and offers a 7 day service, from 8am until 8pm, providing assessment, treatment and evaluation of clients’ needs, in a short term intervention program. The team’s aim is to prevent unnecessary admission to hospital and provide Urgent Community Response.
The post holder will require excellent communication skills to build complex relationships and network with key stakeholders to enable integration. Using initiative is essential, as is skill sharing with colleagues within the team, and the wider organisation. It is essential that all the team members work closely together, to truly integrate an MDT and provide a comprehensive service, utilising the Calderdale framework.
There are opportunities for therapy skills development and further education opportunities. We are committed to supporting CPD through our robust appraisal system. This is supported by in-service training, access to external courses and clinical supervision.
