Locala Health and Wellbeing is recruiting an Occupational Therapist to join the Integrated Transfer of Care (ITOC) Team. This permanent Band 6 role offers 37.5 hours per week, with flexible working and 0800–1800 coverage across 7 days (1 in 4 weekends).
You will work autonomously within a multi-disciplinary team, providing OT assessments, home visits, and equipment prescriptions to support safe hospital discharge and promote independence in the community.
