Locala is welcoming applications from enthusiastic, motivated, and caring experienced qualified nurses to join our Community Nursing Network (CNN) on our Bank workforce. We will support you to thrive in the role and continue to develop your community nursing skills.

Main duties of the job

  • Delivering seamless nursing care to housebound patients.
  • Coordinating care to help prevent unnecessary hospital admissions.
  • Promoting self-care for patients with long-term conditions.
  • Contributing to health promotion and innovative projects that support improved patient outcomes.

Clinical Skills Required

  • Comprehensive wound assessment and wound care management
  • Application and monitoring of compression therapy
  • Administration of medications, including insulin and controlled drugs
  • Catheterisation skills: male, female, and suprapubic care, including troubleshooting
  • End-of-life and palliative care support
  • Management of long-term conditions (e.g., diabetes, COPD, heart failure)
  • Venepuncture
  • Holistic clinical assessment and development of care plans
  • Pressure area assessment and prevention planning
  • Monitoring vital signs and escalation of clinical concerns
  • Accurate documentation and confident use of SystmOne
