The Hospital at Home service offers acute level care at home to Kirklees residents 7 days a week between the hours of 8am and 8pm. The patients will be supported across Kirklees by a dedicated team, with oversight and support from Acute Consultants, GPs and appropriate clinicians. As a key member of the Hospital at Home, you’ll help prevent admission to hospital and support earlier discharge by keeping patients at home.
The service is a multidisciplinary team with local colleagues including Senior Nurses, Physiotherapists and Health Care Assistants. You will work in partnership with clinicians within local Acute Trusts to ensure patients receive a holistic and safe service. You will need a variety of clinical skills to undertake the role and will support the remote monitoring service as part of the Hospital at Home offer.
We are seeking a full time Band 6 Nurse to join our team with an interest in acute support for patients within the service, working alongside our colleagues across Calderdale, Kirklees and Wakefield. A full valid UK driving licence with access to a vehicle and business insurance is essential. The post holder must be able to travel independently across the borough.
