Locala Health and Wellbeing is seeking a Band 6 Occupational Therapist to join the Integrated Transfer of Care (ITOC) Team. You will help people leave hospital safely, regain independence and return to living well in their own homes and communities.
Working in a multidisciplinary team across A&E, Frailty and Recovery Hub, you will deliver person-centred assessments and tailored interventions to prevent unnecessary admissions and to coordinate safe transfers of care.
