An excellent opportunity has arisen to join the 'Family Hubs' service as a Health Practitioner, with 4 new positions within the Calderdale and Kirklees 0-19 services. 2 positions will be based in the Kirklees Gateway, and 2 in the Calderdale Family Hubs Single Point of Contact.

The Family Hubs Health Practitioner (FHHP) will work within a multidisciplinary team and will act as the first point of contact for those reporting concerns about children and young people, and families, in Kirklees and Calderdale. You will have a sound working knowledge of health services that support the family, delivering support and guidance to families and professionals in an engaging, caring and non-judgmental manner.

You will practice professional curiosity when handling calls to achieve a desired outcome, safely and effectively, and handle information in confidence, which at times can be emotive and sensitive in nature. Playing a central role in embedding a "whole family" approach in line with the early help strategy, you will work collaboratively across local authority, health, education, and voluntary sector partners, with an aim to reduce escalation to statutory services through early identification, intervention, and robust multi-agency planning.
