Job summary

An exciting opportunity has arisen for the right candidate to join our Community Respiratory Service as Clinical Lead Respiratory Nurse Specialist based at Fartown Health Centre. Our Community Respiratory Team delivers a community nursing service, Hospital at Home and pulmonary rehabilitation to individuals with chronic lung disease within the Greater Huddersfield area. The team comprises experienced Specialist Nurses, Physiotherapists, Occupational Therapist, Exercise Physiologist and Health Care Assistants. The successful candidate will have an extensive respiratory background and be passionate about leading high standards of care and management for patients diagnosed with chronic respiratory conditions.

• Providing clinical leadership to the nursing team, delivering interventions to avoid unplanned and unnecessary hospital admissions for patients with a respiratory condition




• Providing clinical leadership into the delivery of Hospital at Home for patients with chronic respiratory conditions




• Ensuring high quality, evidence based nursing input for the patients with Chronic Lung Conditions in Greater Huddersfield




• Working alongside the Clinical Lead Band 7 Physiotherapist in delivering high quality care within the Community Respiratory service and the Pulmonary Rehabilitation service
