Bank Band 5 Community Staff Nurse – Join Our Community Nursing Network

Locala is welcoming applications from enthusiastic, motivated, and caring experienced qualified nurses to join our Community Nursing Network (CNN) on our Bank workforce. We will support you to thrive in the role and continue to develop your community nursing skills. We have nine teams across North Kirklees and Greater Huddersfield, delivering high‑quality care to patients in their own homes and local communities.

As a Bank Band 5 Community Staff Nurse, you will play a vital role in delivering person‑centred care, carrying out holistic assessments, and supporting patients to manage their health needs effectively.

Responsibilities:
  • Delivering seamless nursing care to housebound patients.
  • Coordinating care to prevent unnecessary hospital admissions.
  • Promoting self‑care for patients with long‑term conditions.
  • Contributing to health promotion and innovative projects that improve patient outcomes.
