Are you a confident nurse with community experience who wants to work with patients in an acute setting to maximise their independence? An excellent opportunity has arisen for an enthusiastic and motivated nurse to join Locala’s new Self-Management Service. The service aims to support patients in becoming experts in their own healthcare interventions, working towards the NHS’s Long-Term Plan for personalised care. Patients will be reviewed both at home and virtually, with the goal of maximising their self-management skills.
Your main role will be to support learning and competence for patients, ensuring they feel confident and able to optimise their skills. You will be based primarily at Huddersfield Royal Infirmary.
The successful candidate will:
- Be a nurse with community experience passionate about patient choices, their ability to self-manage, and personalised care.
- Build effective relationships with colleagues in the acute setting, developing processes and pathways to encourage the transition from hospital to home.
- Be willing to undertake additional clinical skills and health coaching training as required.
- Act as a role model, supporting patients in an acute setting to build confidence and continue their skills at home.
- Demonstrate flexibility, allowing for regular evaluation and adaptation of the service during its early stages.
- Utilise support services throughout Kirklees to deliver personalised, high-quality care, improving patient outcomes and positively influencing service change.
