Job Role
Responsibilities
- Reducing hospital stay durations for those no longer needing acute care
- Assessing individuals in appropriate settings outside the hospital
- Providing multidisciplinary reablement and rehabilitation plans
- Enabling the urgent care system to prioritize acute hospital care for those who need it most
Service Structure:
- The Hospital Discharge Team identifies individuals with ongoing care needs and arranges discharge pathways within the Recovery hub service.
- Home First and Bedded Pathway is the default discharge route, focusing on returning individuals home with necessary support.
Opportunities for broader practice support across Unplanned and Intermediate Care services are available.
