Locala Health and Wellbeing is seeking an experienced Occupational Therapist (Band 6) to join the Integrated Transfer of Care (ITOC) Team in Huddersfield. The role is permanent and full-time (37.5 hours weekly) with a flexible working pattern across 7 days to support timely hospital discharge.
You will work within a multi-disciplinary team, conducting triage, home assessments, and goal-oriented interventions to maximise independence and safety for service users.
