Hours per Week: 37.5 Per Week – The service runs 0800-1800 across 7 days (1 in 4 weekends) with flexible working hours.

Responsibilities

An excellent opportunity has arisen to join our Integrated Transfer of Care (ITOC) Team – multi-skilled team consisting of nurses, occupational therapists, physiotherapists, and Assistant Practitioners. We also work in alignment with social services and other Locala community services.

The ITOC team work across multiple service previsions and bases in the Kirklees district providing clinical triage within the acute setting, in-reach therapy to a community Recovery Hub facility, and complete frailty and falls assessments within our service users own environments.

Our purpose is to support a safe and timely hospital discharge of people who no longer need acute hospital care, to a place more suitable to the person’s needs. Assessments and care provision can then be tailored to support people to regain strengths and skills so that they can live as independently as possible. We also aim to avoid admission for patients in A&E and Frailty departments who require intervention to allow them to be discharged home.

You will be working autonomously as well as part of a multi-disciplinary team, liaising with health and social care colleagues to coordinate services, promote recovery from illness, and enable health, wellbeing and self-care. You will be visiting service users within their own homes using goal orientated focus, to maximise their independence, safety and quality of life. We have a robust lone working policy with lone working devices to support the safety of our employees.

Therapy colleagues in the team also provide in-reach services at our Moorlands Grange Recovery Hub in South Kirklees, this is a social service funded short term placement setting for service users who no longer need the acute medical care but require further assessments either due to acute delirium or are non-weightbearing following a fracture and/or other surgery or other complex health/care needs.

The main priority of the team is to consider home first with ongoing community interventions, if required.

OTIC Model of Care

  • Reducing the time people spend in hospital when they no longer need acute care preventing hospital acquired infections and ‘deconditioning’ (the loss of strength and independence)
  • Assessing people in a more appropriate environment than the hospital giving a more accurate indication of their strengths and needs
  • Providing multidisciplinary reablement and rehabilitation plans, and if necessary short-term care and support, to help people gain and re-gain independence, preventing or reducing need for longer term care.
  • The model also enables the urgent care system to prioritise acute hospital care for those people who need it.

The ITIC teamwork in partnership with individuals and families to identify their own needs and short-term goals, recognising that person-centred care planning and intervention is key to the person accomplishing the outcomes they want to achieve. Occupational Therapists provide therapy assessments and equipment for the patients in A&E, Frailty and the Recovery Hub. We also support those that need to gain basic physiotherapy competencies, patient vital observation competencies and support with all aspects training needed for the ITIC service including triage and reviewing complex patients.
