Responsibilities
Are you looking for a new, exciting challenge? The Hospital at Home service was commenced from October 2022 (formally known as Frailty virtual Ward). This service supports patients with acute needs who can have their acute care provided in the community instead of having their care within a hospital setting. The Hospital at Home service offers acute level care at home to Kirklees residents 7 days a week between the hours of 8am and 8pm. The patients will be supported across Kirklees by a dedicated team, with oversight and support from Acute Consultants / appropriate clinicians.
As a key member of the Hospital at Home, you’ll be part of our continuing success in Kirklees, preventing admission to hospital and supporting earlier discharge by helping keep patients at home. The service works as part of a Calderdale, Kirklees and Wakefield (CKW) approach. The Hospital at Home service is a multidisciplinary team with Locala colleagues including Frailty Nurses, Physiotherapist and Health Care Assistant. They will work in partnership with clinicians within the local Acute Trusts to ensure patients have a holistic and safe service offer.
You will need a variety of clinical skills to undertake the role such as phlebotomy and clinical observations. You will also support the remote monitoring service as part of the Hospital at Home offer. We are seeking a Health Care Assistant to join our new team with an interest in acute support for Frailty patients within the service, working alongside our colleagues across Calderdale, Kirklees and Wakefield. The position is to cover a maternity leave of one of our existing HCA’s.
A full valid UK driving licence with access to a vehicle and business insurance is essential for the role.
