An exciting opportunity has arisen for a motivated and enthusiastic Band 3 Health Care Support Worker to work in the Locala Community Cardiac Rehabilitation Team in Huddersfield. The post holder will be required to work within the service, supporting excellent holistic care to patients receiving cardiac rehabilitation. This will involve working with the multi-disciplinary team of nurses, physiotherapists, and exercise professionals in a wide variety of community settings.

You will be part of the team running the Cardiac Rehabilitation programmes, including:

  • Setting up and supporting patients in exercise classes
  • Education sessions
  • Assessments and home programme delivery
  • Working in the nurse led clinics taking patient observations and histories

There will be some administrative duties to undertake on behalf of the team. We are looking for an individual with multi-disciplinary skills who is confident working as part of a team in a variety of settings. Experience of working with patients with cardiac disorders would be ideal but not essential as support and training will be offered.

Key Responsibilities:

  • Be part of the team delivering Cardiac Rehabilitation
  • Work closely with the Physiotherapist and Exercise Physiologists to support assessments and clinic delivery
  • Work closely with the Cardiac Nurses to support clinics by taking baseline observations and simple histories
  • Support the delivery of clinics by booking in patients, arranging appointments, ensuring adequate equipment and supplies

Requirements:

  • High level of interpersonal skills for working face to face with patients and carers
  • Experience/good knowledge of working in community settings
  • Ability to work within a multidisciplinary team
  • Evidence of good communication and team-working skills, including some basic administration skills
