Responsibilities

An excellent opportunity has arisen for a motivated and enthusiastic Band 4 Assistant Practitioner to work in the Locala Community Respiratory Team in Huddersfield.

The post holder will be required to work within the service, providing and supporting excellent holistic care to our chronic respiratory patients. This will involve working with the multi‑disciplinary team of nurses, physiotherapists, occupational therapists and exercise professionals in a wide variety of community settings.

You will be part of the team running the Pulmonary Rehabilitation programmes, including classes, education, assessments and home programme delivery as well as working in the wider team undertaking home visits to deliver key elements of care (equipment provision, rehabilitation and self‑management support). You will also have the opportunity to be involved in development and improvement of the services.

We are looking for an individual with multi‑disciplinary skills who is confident working in community. Experience of working with respiratory patients would be ideal but not essential as support and training will be offered.

  • Be part of the team delivering Pulmonary Rehabilitation - group sessions and home activity programs.
  • Work closely with the Occupational Therapist & Physiotherapists, to undertake assessments, implement treatment plans and support self‑management skills for patients and their carers’
  • Provide holistic care including equipment provision, general rehabilitation, and opportunity to develop skills in symptom management programmes.
  • Be an experienced assistant practitioner with the skills to provide assessment, planning & implementation of treatment programmes with patients with respiratory conditions with support of clinical colleagues.
  • Have experience/good knowledge of working in community settings.
  • Have the ability to work with patients and significant others to support personal change management strategies.
