As a member of Liverpool Carers Centre, you will identify unpaid carers and support them to access practical and emotional support. You will aid in empowering carers to take control of their own health and wellbeing, focusing on the 'what matters to me' philosophy and taking a holistic approach to connect carers to community groups and statutory services.
Core Responsibilities:
- Support carers to access the most appropriate services to assist them in their caring role.
- Develop relationships that help and support unpaid carers while being a main enabler for holistic solutions.
- Car owner/driver is essential.
Accountable for:
- Developing and sustaining Health & Wellbeing Support.
- Supporting collaborative working.
- Providing advice, guidance, and practical support in the implementation of support packages.
- Being one of the key contacts for the Health & Wellbeing project for Local Solutions, particularly around GP Surgeries and Health Care.
Responsibilities:
- Work with GP practices and Health Care settings to raise awareness and identify unpaid carers.
- Promote the pathway of receiving referrals from GP surgeries regarding carers.
- Support carers to access relevant support depending on their needs.
- Give presentations, attend meetings, and provide information to GP practice staff and other organisations.
- Be part of the health and wellbeing team within the carers centre and participate in events and activities that offer support to carers.
- Complete administrative tasks using Excel and other software systems.
- Use a variety of social media platforms to help raise awareness.
- Provide monthly KPIs and reports as required.
- Take on other roles that will support this piece of work.
Accountabilities:
- Work and support GPs and Health Care to raise awareness and recognition of unpaid carers and implement support to meet carers' needs.
- Work to targets on the number of referrals received as a result of the support provided.
- Ensure new referral information is added accurately to databases and systems.
- Adhere to data protection legislation and data sharing agreements, ensuring that all carers supported by the service are accurately recorded using agreed templates.
- Complete monitoring as required to demonstrate outcomes of the service.

- Perform any other duties appropriate to this post, as necessary or as requested.
- Participate in professional development, using appropriate learning opportunities to improve relevant knowledge and skills.
- Ensure that all activities of the project provide equality of opportunity.
General Skills and Experience:
- Experience of working in health or social care.
- Experience of supporting unpaid carers or collaborating with people.
- Ability to work as part of a team while prioritising your own workload and managing your own diary.
- Build relationships with both carers and professionals, manage databases, and take referrals.
- Attendance at occasional evening or weekend events.
- Technical expertise should include the ability to use Microsoft Office, Excel, and Outlook.
The successful candidate will require an enhanced Disclosure & Barring Check and undertake mandatory safeguarding training as appropriate for the role. Travel is required, so a car owner/driver is essential.
