We are excited to be hiring a new Team Manager to join our Reablement team. The team is based in Woking and offers hybrid working. There is an expectation to work from the office regularly (typically at least two days per week) to promote collaboration with colleagues and partners. Frontline services operate 7 days a week; leadership cover will reflect operational needs.
Our Reablement Service provides time limited support (typically up to six weeks), usually in people’s own homes, helping people work towards their reablement goals so they can live more independently and stay in their communities. We work as a therapy led, multi disciplinary team alongside Team Leaders, Senior Reablement Practitioners, Reablement Practitioners and Therapists. Hybrid working is supported, and our frontline services operate seven days a week, 365 days a year.
This is a pivotal leadership role in a CQC registered service, providing operational leadership and professional oversight to ensure safe, effective, person centred delivery of regulated reablement activities. You’ll drive quality assurance and compliance with statutory standards, strengthen workforce capability through supervision and performance management, and support system wide collaboration with NHS and community partners to coordinate integrated, multidisciplinary care. The role covers a geographical area, with flexibility to work across other localities temporarily to support service resilience. You will report to a Reablement Senior Manager and help shape service performance, operational resilience, and improvement across our reablement pathways-supporting timely hospital discharge, prevention of admission, and promoting people’s independence at home.
Day To Day Duties Include:
- Leading a locality reablement team delivering regulated activity within a CQC registered service; provide day to day operational leadership, professional oversight and robust quality assurance.
- Ensuring compliance with statutory and regulatory requirements; monitor quality, respond to risks and issues, and maintain safe, person centred practice.
- Planning and organising resources to meet demand across your geography; flex support across other areas when needed to maintain service continuity.
- Working in partnership with health professionals and system partners to support joint care planning, discharge coordination and multidisciplinary approaches.
- Leading on complex operational challenges, including safeguarding and high risk situations, ensuring appropriate action and escalation.

Your Application:

- Strong understanding of CQC regulated activity, relevant legislation, procedural frameworks and social care practice standards.
- Proven ability to lead, supervise and develop staff, manage performance, and support consistent practice standards across a locality.
- Excellent communication, negotiation and influencing skills and the confidence to engage with partners, staff and the public in challenging situations.
- Competent user of IT systems (e.g., Microsoft 365 and care recording systems).
Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.
