Local Care Direct is a non-profit Social Enterprise operating 24/7 to provide primary and urgent health care services across West Yorkshire.
Salary: £29.02 per hour - £37.73 out of hours
Location: Local Care Direct, Unit 2, Longbow Close, Pennine Business Park, Huddersfield, HD2 1GQ
Shifts Available: Shifts available across a 4 week rolling rota
Main duties of the job
The advanced clinical practitioner will support the Urgent Community Response (UCR) services mission to deliver interventions within a 2-hour timeframe, either through face-to-face visits by partner clinicians or remote management. The advanced clinical practitioner will be an integral member of the clinical team to deliver high-quality patient care, involving the use of technology to conduct remote consultations for patients referred into the service. The advanced clinical practitioner will have a strong interest in complex triage and is skilled in remote consultations and confident in managing acutely unwell patients. The advanced clinical practitioner will strive to make a real difference by helping patients stay independent and avoid unnecessary hospital admissions.

Experience
- Evidence of a range of clinical experience, working independently in a healthcare role.
- Evidence of experience of triage, by telephone or face-to-face
- Continued professional and personal development.
- Experience of mentorship, leadership & research in clinical practice.
- Experience of working in more than one clinical setting.
- Minimum of 1 year experience in practice post qualification.
- Experience of working in primary/urgent care.
- Experience of managing paediatrics
- Experience of managing minor illness.
- Experience of managing chronic disease.
- Experience of managing geriatrics.
- Experience of telephone/video remote consultation.
Qualifications
- One of the following: 1st level Registered Nurse or equivalent professional qualification/registration with a professional body, MSc/Post Graduate diploma in Advanced practice, Paramedic HPC registration MSc/Post Graduate diploma in Advanced practice or Pharmacy GPhC Registration MSc/Post Graduate diploma in Advanced practice
