Commissioning Nurse
Join the LNL-INFN team to support individuals with learning disabilities and mental health needs in Leicester, Leicestershire and Rutland.
Key responsibilities include:
- Conduct comprehensive, person‑centred assessments for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) eligibility.
- Collaborate with local partners, including health, social care and independent providers, to plan, manage and review patient care needs.
- Provide clinical advice and support in relation to NHS Continuing Care and FNC.
- Lead and mentor staff to deliver a high‑quality service and drive continuous improvement.
- Progress the delivery of care under the Personal Health Budget agenda.
- Support the Personalised Healthcare Commissioning team to ensure workstream plans are fulfilled.
- Maintain professional registration and up‑to‑date knowledge of legislation.
- Contribute to performance improvement within the team of Senior Practitioners, Practitioners and Associate Practitioners.
- Communicate information, risks and dependencies, including briefings and reports.
Qualifications and experience:
- Good understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.
- Excellent communication, problem‑solving skills and proven ability to produce high‑quality documentation under deadlines.
- Experience working autonomously and collaboratively in a multi‑agency environment with individuals and representatives.
- Internal/CSU candidates will take preference.
