Responsibilities:

  • Supporting families across Monmouthshire who are affected by domestic abuse and struggling to maintain their tenancies as a result.
  • Undertake assessments of need in accordance with the support assessment procedure and as directed by the Project's Manager.
  • Undertake risk assessments and risk management plans, including Safelives DASH risk indicator and safety planning in accordance with the policy of Llamau.
  • Work within a multi‑agency Hub in Monmouthshire, liaising with colleagues to provide timely, appropriate support.
  • Work within the Supporting People Gateway framework, liaising with staff as required for case management and allocation.
  • Deliver support and assistance to each family in accordance with the ethos, values, and policies of Llamau.
  • Ensure that all appropriate actions are taken to prevent unnecessary tenancy breakdown.
  • Work with partners to address any related needs impacting on their tenancy – this could include DWP, housing providers, Housing Benefit, courts, and workplaces.
  • Ensure that all support notes are completed at the end of each session and that all case management notes are completed, as per Llamau policy.
  • Ensure that each family builds and sustains an informal community support network that can enable that family to live successfully within their community.
  • Ensure liaison with relevant agencies that may be working with the individual, e.g. Social Services, Police, IDVA, etc., and ensure appropriate referrals are made to specialist or statutory agencies where required.
  • Undertake assessments of home security, liaising closely with Target Hardening Schemes, housing providers, and other relevant agencies.
  • Support families to access alternative accommodation if required.
  • Ensure that accurate monitoring is completed for each Service User.
  • Ensure Pan Connect and Locata electronic database systems, including the time entry system, are completed in accordance with the policies and procedures of Llamau.
