Support Worker - Radcliffe On Trent, Nottingham, NG12
We are seeking dedicated, compassionate, and reliable Support Workers to join our team. You will provide person-centered support to individuals with learning disabilities within their own homes, accompany them to day centers alongside support staff, and assist them in accessing the local community to promote independence and social inclusion.
Main Duties and Responsibilities
Assist individuals with morning, evening, and personal care routines, meal preparation, and everyday household tasks.
Escort and support service users to and from day centers, working alongside day center staff and care teams.
Accompany individuals into the community for social events, recreational activities, appointments, and shopping trips.
Encourage skill development, confidence, and personal autonomy in line with individualised support plans.
Administer medication accurately and complete clear, compliant daily record-keeping.
