Mental Health Team Leader - Supported Living
Living Horizon provides specialist supported living for adults with mental health needs in Aylesbury, helping residents build stable, more independent lives.
We are looking for an experienced Team Leader (House Leader) to lead one of our Aylesbury homes. You will lead and supervise a small team of Support Workers, make sure the home runs safely and to standard, and be the main point of contact for residents, local authority contacts, care coordinators and other professionals.
Your role includes:
- - Leading, supervising and supporting the Support Workers in the home
- - Writing and reviewing care plans and risk assessments
- - Keeping our digital care records up to date and completing thorough handovers
- - Safeguarding residents and reporting concerns straight away
- - Investigating incidents and putting improvements in place, with management support\
- - Working with health professionals and attending meetings at the house
- - Organising the day-to-day running of the home, including cleaning and maintenance arrangements

- - At least 4 years' experience supporting adults with mental health needs
- - Experience leading or supervising staff (for example as a senior support worker, team leader or manager)
- - Supported living or care experience
- - Good organisation and IT skills (we use online systems and email)
- - Clear, compassionate communication with residents, staff and professionals
- - Right to work in the UK
