Livewell Southwest is seeking a Band 6 Occupational Therapist to join the Community Crisis Response Team in Plymouth. You will manage a caseload of acutely unwell patients at home, deliver holistic OT assessments, and work within a large MDT to prevent hospital admissions.
You will also educate junior staff, contribute to service development, and liaise with statutory and voluntary bodies to ensure safe, effective home-based care.
