The predicted salary is between 43200 - 72000 £ per year.
An exciting opportunity has become available for a highly experienced, health &social care leader, responsible for the delivery & development of health &social care community services across Plymouth &West Devon.
This senior role, reports to the Chief Operating Officer &works closely with the Board &Executive Team, driving the delivery of outstanding health &care services, to a predominantly older population &particularly people living with Frailty at risk of losing their long-term independence.
You will hold relevant clinical or professional qualification (Eg, NMC, HCPC, SW England) &will have held senior leadership positions in a provider organisation with responsibility for the management of people, facilities&finances & a demonstrable track record of delivering&leading through change.
In addition, you will bring knowledge of the current strategic context &will help to create a clear vision for the expanded role that community services play in ensuring that an increasing number of people experience high quality treatment &care at home. Working in partnership with our acute provider, as our well as primary care networks, local authorities &VCSE sector organisations.
Working alongside two other senior leaders, with responsibility for children, young people & families & mental health, neurodiversity & wellbeing services respectively, who collectively are responsible for developing integrated models of care that deliver joined up care, closer to home.
A highly motivated & self-driven individual, with an ability to proactively seek out & take advantage of new opportunities & relationships.

Will be a strong advocate for new technologies & the increasing role that digital tech will have play in enabling people to enjoy more years of good health, whilst also enabling us to delivery care more efficiently through remote monitoring, video assessment & going forwards, Artificial Intelligence.
As someone with clinical experience, you will also use your professional expertise & knowledge to influence positive change; as such you will be able to speak with authority on topics like reablement, rehabilitation & recovery, community nursing interventions, frailty & social care.
