Join LIVES and play a pivotal role in transforming urgent and emergency care across Lincolnshire. As a Community Emergency Medicine ACP, you will work autonomously and as part of a multidisciplinary team, delivering advanced assessment, diagnosis, treatment and clinical decision-making for patients' homes and community settings. This is a dynamic frontline role focused on providing the right care, in the right place, at the right time.
You will manage a wide range of undifferentiated presentations, support virtual wards, utilise community pathways and help patients avoid unnecessary hospital attendance or admission wherever it is safe to do so. Working across pre-hospital, urgent and community care environments, you will make a tangible difference to patient outcomes while supporting prevention, early intervention and care closer to home.

Main duties of the job include:
- Delivering advanced assessment, diagnosis, treatment and clinical decision-making for patients in community and pre-hospital settings across Lincolnshire.
- Managing a wide range of undifferentiated presentations and supporting virtual wards.
- Providing safe, effective care closer to home through holistic assessment of patients' medical, functional and social needs.
- Identifying risks, supporting early intervention and safeguarding, making referrals to appropriate services, and empowering patients and carers to make informed decisions about their care.
- Operating as part of a mobile clinical team, responding to incidents across the county, undertaking dynamic risk assessments and maintaining accurate clinical records.
- Working collaboratively with urgent and emergency care partners, primary care, social care and specialist services.
- Contributing to clinical governance, service improvement, education and mentorship.
- Supporting the charity's mission through community engagement, awareness raising and fundraising activities.
