The role:

We are looking for a motivated and solutions focused Hospital Discharge Assessment & Prevention Officer to join our Housing Solutions Service. Working closely with customers, hospital discharge teams and partner agencies, you will identify people who are homeless or at risk of homelessness and take early, proactive action to prevent homelessness wherever possible. This will involve completing housing and homelessness assessments and developing practical, sustainable housing solutions. Prevention will be at the heart of your work. This may mean negotiating to save a tenancy, addressing barriers to someone returning home, working with family or support networks where appropriate, resolving affordability issues, coordinating support, or identifying a suitable alternative housing option. We are looking for someone with strong assessment, negotiation and problem-solving skills, who can work creatively with customers and partners to achieve positive outcomes.
