Liverpool City Council is looking for a dedicated and experienced Social Care Assessor to join their Adult Social Care & Health Team. This is a fantastic opportunity to work within a strengths-based, reablement-focused approach that places the person at the centre of care.

Your Role:

As a Social Care Assessor, you\’ll be part of a professional and supportive team working to make a real difference in people\’s lives. You will:

  1. Carry out strengths-based assessments with adults and carers
  2. Develop and review outcome-focused support plans that promote independence
  3. Apply a reablement-first approach in all interventions
  4. Collaborate with families, health professionals, and partner agencies
  5. Manage your own caseload effectively and meet review timelines
  6. Maintain accurate records using Liquid Logic case management system
  7. Work across multi-disciplinary teams to achieve the best outcomes for service users

Requirements:

  • Experience in social care and reablement settings
  • A relevant social care qualification
  • Proven experience working in a local authority or statutory/voluntary agency
  • Confidence using Liquid Logic
  • Strong understanding of adult social care legislation and safeguarding
  • Excellent communication and interpersonal skills
  • Ability to build effective partnerships and tailor support to individual needs
