As a Social Care Assessor on the Hospital Discharge Team, you will support adults (18+) to leave hospital safely and regain independence. Your responsibilities include completing Care Act assessments, applying Prevent, Reduce, Delay principles, managing a varied caseload, and collaborating closely with nurses, therapists, and other health professionals.
Key Responsibilities
- Complete Care Act assessments, including Needs, Risk, Mental Capacity and Best Interest decisions
- Use strengths‑based practice to deliver flexible, outcome‑focused support
- Apply Prevent, Reduce, Delay principles to minimise long‑term care dependency
- Work on‑site as part of a Multi‑Disciplinary Team (MDT), with occasional home visits
- Collaborate closely with nurses, therapists and other health professionals
- Manage a varied caseload, prioritising early intervention and short‑term support
- Commission services including reablement, domiciliary care, assistive technology and carer support
- Take part in MDT meetings, duty functions and shared decision‑making
- Ensure all work is compliant with legislation, policy and best practice
