Responsibilities
- Lead on supporting children with SEND within one of our children’s centres.
- Provide direct support to children and families with additional or complex needs.
- Work with parents to plan and deliver targeted play and learning activities.
- Identify needs early and offer appropriate support and interventions.
- Offer targeted support including home visits and parenting support where needed.
- Support children’s speech, language and communication development.
- Work with partner agencies (health, therapy, social care) to coordinate support.
- Contribute to early help assessments and act as Lead Professional where required.
- Signpost and support families to access funding and additional support (e.g. DLA).
- Promote inclusion across all services and activities within the centre.
- Support the delivery of specialist sessions with external professionals (e.g. SALT, OT, CAMHS).
- Maintain accurate, up‑to‑date records on systems (e.g. Liquidlogic/EHM).
