Job Introduction

Job title: Social Care Assessor – City Wide

As a Social Care Assessor, you will be responsible for providing high quality support to adults in need of social care across the city. We are seeking a professional and dedicated individual to join an established Locality team at Liverpool City Council. Responsibilities

  • Contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated team responsible for safeguarding, assessment, support planning and promoting the wellbeing of adults with care and support needs, and continuously improving outcomes for people
  • Apply the Prevent, Reduce, Delay principles of the Care Act to promote independence and wellbeing in every interaction
  • Embed a strength‑based approach to assessment and support planning with outcomes that people want to achieve
  • Plan and co‑design individual support plans with people
  • Ensure that regular monitoring and reviews of support plans are carried out
  • Manage your own caseload whilst maintaining the reputation of the Department and Council and complying with national and service procedures and guidance
