The role: As a Senior Practitioner within the Hospital Discharge Team, you will provide professional leadership, line management and mentoring to Social Workers and Social Care Assessors working within a fast‑paced, multidisciplinary hospital environment. You will manage a small but complex caseload and support the delivery of safe and timely discharges.
As a Senior Practitioner you will ensure the delivery of high‑quality, strengths‑based assessments and outcome‑focused support planning that enable people to return home safely wherever possible, prevent unnecessary admissions to long‑term care, and reduce delayed discharges. You will support timely assessments, reviews and decision‑making, balancing risk, proportionality, and person‑centred outcomes.
The role as a Senior Practitioner requires strong operational oversight, including practice supervision, coaching and quality assurance in a hospital setting. You will support staff to complete Mental Capacity Act assessments, best‑interest decision‑making, safeguarding enquiries, and complex discharge planning, including cases involving CHC, Section 117 aftercare, homelessness, substance misuse, and family disagreement.
As a Senior Practitioner you will work closely with acute trust colleagues, ICB partners, therapy services, discharge hubs, trusted assessors, and community teams to resolve barriers to discharge. This includes chairing and contributing to multidisciplinary meetings, supporting escalation and resolution of system pressures, and ensuring discharge plans are legally robust, proportionate, and defensible.
You will also contribute to performance management, financial oversight, and value‑for‑money decision‑making within Adult Social Care. In this role as a Senior Practitioner, you will prepare and quality‑assure written work, including professional reports, complex case records, and Court of Protection documentation where required, and support staff to evidence sound professional judgement and reflective practice in high‑risk environments.
As a Senior Practitioner you will model inclusive, strengths‑based and trauma‑informed practice, aligned with the Council’s values and Competency Framework (Level 2). You will foster a culture of learning, accountability and continuous improvement, supporting staff development while maintaining high standards under pressure.
This role as a Senior Practitioner is critical to maintaining strong collaborative relationships with health colleagues and partners, embedding consistent hospital discharge standards, and supporting whole‑system resilience during periods of high demand. While the Senior Practitioner will have a designated base, the role requires flexibility to work across hospital sites in response to service demand.
