Responsibilities:
- Lead a team of Brokerage Officers and broker services to find the right care and support solutions for people.
- Work within a portfolio area (Transitions, Working Age Adults or Older Adults) and partner with the Adult Social Care and Health Directorate’s Commissioning Team, social care assessment staff, NHS colleagues and care providers to arrange, coordinate, negotiate and broker packages, placements and supporting social care services in response to identified needs.
- Shape and develop portfolio expertise and streamline brokerage processes to support service delivery.
Qualifications & Experience:
We are particularly interested in candidates with a background in children’s or young adults service provision, social care, brokerage or placement finding experience; candidates with adult social care experience are also welcome.
