The SEND Link Worker supports children with SEND within one of our children’s centres, working directly with families to plan and deliver play and learning activities, and collaborating with partner agencies to coordinate support.

Responsibilities

  • Lead on supporting children with SEND within a children’s centre
  • Provide direct support to children and families with additional or complex needs
  • Work with parents to plan and deliver targeted play and learning activities
  • Identify needs early and offer appropriate support and interventions
  • Offer targeted support including home visits and parenting support where needed
  • Support children’s speech, language and communication development
  • Work with partner agencies (health, therapy, social care) to coordinate support
  • Contribute to early help assessments and act as Lead Professional where required
  • Signpost and support families to access funding and additional support (e.g. DLA)
  • Promote inclusion across all services and activities within the centre
  • Support the delivery of specialist sessions with external professionals (e.g. SALT, OT, CAMHS)
  • Maintain accurate, up-to-date records on systems (e.g. Liquidlogic/EHM)
