Home Health Personal Care Assistant
Provide personal and basic nursing care to clients in their homes according to an established plan of care. Assist with hygiene, transfers, mobility, meals, and companionship while promoting comfort, dignity, and safety.
Key Duties
- Deliver hands-on care aligned with the plan of care.
- Assist clients with daily living activities.
- Support mobility and safe transfers.
- Provide light housekeeping as needed.
- Monitor and observe client well-being.
- Document services provided and report any changes in condition.
- Travel within the assigned service area to reach clients’ homes.
Qualifications / Skills
- Compassionate, client-centered approach and ability to maintain dignity.
- Basic nursing and personal care knowledge.
- Skills in mobility support, safe transfers, and hygiene assistance.
- Attention to detail for documentation and communication.
- Ability to perform light housekeeping tasks.
- Reliable travel within the service area.
