Since our inception in 2015 in Denmark, Liva has been a pioneer in the field of digital behaviour change. Our ambition is to empower 1 million people by 2030 to improve their health and well-being by preventing and managing chronic conditions through digitally-powered, evidence-based lifestyle interventions.

These particular roles are required for Momenta Newcastle Limited, who are part of the Liva Group. Momenta Newcastle is a leading UK provider of evidence-based health behaviour change programmes. Commissioned by NHS England, we deliver the 'Type 2 Diabetes Path to Remission Programme' across multiple regions.

We’re looking for a number of in-person Health & Wellbeing Coaches to work with us on a freelance project basis to help deliver our 1:1 NHS T2DR sessions in person, supporting participants through a structured year-long journey toward Type 2 diabetes remission.

  • NENC (Darlington & Stockton, Newcastle & Sunderland)
  • South Yorks (Rotherham & Barnsley)
  • Nottinghamshire (Nottingham, Mansfield, Worksop)
  • Greater Manchester

What you’ll be doing:

  • Delivering one-to-one sessions in local health or community venues
  • Building rapport and tailoring support to participant needs
  • Supporting long‑term behaviour change through structured coaching
  • Documenting all sessions and tracking outcomes

What You'll Have:

  • Experience with health coaching or health interventions
  • A relevant qualification - either a degree in a health‑related subject or a relevant UK health coaching qualification
  • Warm, empathetic and engaging communication style
  • Ability to support a diverse range of patients through structured programme delivery
  • Proficiency with handling patient data and using digital systems and technology
  • Willingness to travel within your region to deliver clinics
  • Able to join up to 3 days training online prior to starting
  • Registered as self‑employed and relevant insurance to deliver health coaching to UK‑based clients

Desirable:

  • Experience delivering 1‑1 health coaching
  • Understanding of type 2 diabetes
