What will your responsibilities be?
- Engage in strengths‑based conversations with service users, families, and advocates to determine eligibility and support planning.
- Undertake Capacity and Best Interest Assessments in line with the Mental Capacity Act 2005.
- Conduct NHS Continuing Healthcare checklists and participate in Decision Support Tool (DST) meetings.
- Manage a caseload effectively within a fast‑paced environment.
- Collaborate with a multi‑disciplinary team, including health, pharmacy, housing, Age UK, and Carers Wakefield.

Qualifications and Experiences
- Social Work England registration.
- Eligibility to work in the UK.
- Qualification in Social Work at degree level.
- Post-qualified experience.
- Relevant experience working with adults.
- Thorough understanding of the Care Act 2014 and the Mental Capacity Act 2005.
- Experience in undertaking Capacity and Best Interest Assessments.
- Familiarity with NHS Continuing Healthcare, including checklists and DST meetings.
- Proven ability to manage a caseload effectively within a fast‑paced environment.
- Strong communication, organisational, and interpersonal skills.
