Liquid Personnel is currently recruiting for a Social Worker to join a Mental Health Hospital Discharge Team based in Stockport.
Responsibilities:
- Join an all-age adult mental health hospital discharge team, collaborating with Pennine Care colleagues.
- Support timely and safe hospital discharges for adults with functional or organic mental health diagnoses.
- Collaborate with individuals, carers, health professionals, advocates, and third-sector organizations.
- Conduct Care Act assessments, develop support plans, perform risk assessments, and carry out Mental Capacity Assessments and Best Interests Meetings.
- Prepare Social Circumstances Reports and attend Mental Health Tribunals to ensure comprehensive support and advocacy for those in your care.
