Liquid Personnel is seeking a dedicated qualified social worker to join its client's busy older persons team in Nottinghamshire.
Responsibilities:
- Manage a complex caseload, conducting assessments, support planning, and reviews to meet individual needs and promote independence.
- Identify opportunities for Reablement services and use a Strength-Based Approach to support well-being and delay long-term care.
- Contribute to Continuing Healthcare (CHC) assessments, coordinate service provision, and ensure effective outcomes.
- Identify community resources and maximise individual assets to support preventative strategies.
- Liaise with providers, offer professional advice to individuals and carers, and undertake safeguarding responsibilities as a Safeguarding Officer.
Qualifications & Experience:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving license
- Post-qualified experience
- Degree level or equivalent in Social Work
