Job Description:
Liquid Personnel is recruiting experienced Social Worker for its client's Older Adults Team at Waltham Forest.
You’ll be working with individuals aged 65+ with a range of needs, including physical frailty, dementia, and long-term conditions.
What will your responsibilities be?
- Undertakestrengths-based assessmentsand reviews
- Develop and reviewcare and support plans
- Work closely with health professionals, families, and community services
- Apply theCare Act 2014,Mental Capacity Act, andsafeguarding procedures
- Promote independence and person-centred outcomes
- Use Mosaic / Liquidlogic (or relevant case management system)

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
