Overview: Liquid Personnel is seeking an experienced Adult Social Worker to join the Mental Health Team of our client. The role involves reviewing supported living placements and collaborating with health partners.

Responsibilities:

  • Evaluate patients' mental health status, identifying needs, strengths, and areas for intervention.
  • Collaborate with the healthcare team to create individualized treatment plans that address patients' mental health needs and goals.
  • Respond to mental health crises, providing immediate support and intervention to stabilize patients.
  • Attend interdisciplinary team meetings and case conferences to discuss patient care and collaborate with other healthcare professionals.
