Liquid Personnel is recruiting for an Adult Social Worker to join the fast-paced Hospital Discharge Team in Darlington. This is a fantastic opportunity to make a real difference by supporting timely, person-centred discharges and empowering individuals to regain independence in their communities.
Responsibilities:
- Support hospital discharges by reviewing individuals placed in temporary residential care following discharge.
- Use a Home First approach that prioritises recovery at home.
- Apply a strengths-based model and person-centred planning in line with the Care Act 2014 to assess needs, promote independence, and coordinate timely, safe transitions from hospital to community settings.

Qualifications & Experience:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
