Liquid Personnel is currently recruiting for a Social Worker to join a Hospital Discharge team based in Halton.
Responsibilities
- Facilitating safe and timely hospital discharges, especially during the challenging winter months.
- Managing a complex caseload independently and efficiently, ensuring that each case receives the attention it deserves.
- Providing crucial support for individuals and families transitioning from hospital to community care, helping them navigate this critical period with ease.
- Collaborating with multidisciplinary teams to ensure seamless, person-centred care, making sure that every individual’s needs are met comprehensively.
- Maintaining a proactive and compassionate approach, dedicated to improving the lives of those you serve.
