What will your responsibilities be?
In this role, you will support individuals aged 18+, handle transitional cases and handover work with children services on a case‑by‑case basis. You will work with adults in the community, hospital, rehabilitation, and care settings within Calderdale, as well as support individuals placed in care settings outside of Calderdale. Operating separately from the Community Mental Health Team (CMHT), you will collaborate with partner agencies for adult social work intervention when needed or requested. You will respond to high demand and provide support during periods of high intensity, including urgent safeguarding work, crisis intervention, duty work, and hospital discharges.

Qualifications & Experience
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have at least 2 years of post‑qualified experience in social work.
