Liquid Personnel is seeking an Occupational Therapist to join the Discharge to Assess (D2A) team in Slough on a hybrid basis.
You will screen hospital discharges, apply clinical judgement, and help ensure patients move safely to the right setting.
You will work with a multidisciplinary team, support timely discharges, and contribute to improving patient outcomes.
HCPC registration and hospital discharge or community rehab experience are required.
