What will your responsibilities be?
- Complete OT assessments and develop reablement plans.
- Support service users to regain independence at home.
- Recommend appropriate equipment and adaptations.
- Work alongside families, carers and other professionals.
- Review outcomes and maintain accurate records.
Qualifications & Experience
- Relevant Occupational Therapy qualification.
- Current HCPC registration.
- Experience in reablement, community or social care.
- Strong assessment and care-planning skills.
- Senior-level experience is desirable.
