Join our client's dynamic Hospital Team delivering high-impact occupational therapy interventions that promote independence and support safe, person-centred hospital discharges. This is a hybrid role with opportunities to work across London hospitals and in the community.
Responsibilities
- Assess Hackney residents in hospitals across London for safe discharge planning
- Collaborate with multi-disciplinary teams and attend ward meetings
- Support residents with complex needs, including LD, MH, dementia, and NRPF
- Conduct community reviews and Care Act assessments
- Promote co-production and independence in daily living

Qualifications & Experience
- HCPC-registered Occupational Therapist
- Experience in hospital or social care settings
