Liquid Personnel is recruiting an Adult's Social Worker to join its client's Community Team, based in Barking and Dagenham.
Responsibilities
- Manage a caseload of adults with varying levels of need, including complex cases depending on experience.
- Complete Care Act assessments, reviews, and reassessments.
- Develop and review person-centred support plans that promote independence and wellbeing.
- Undertake safeguarding enquiries and manage risk effectively.
- Complete Mental Capacity Assessments and apply the Mental Capacity Act where appropriate.
- Support hospital discharges and work proactively to prevent unnecessary admissions.
- Work collaboratively with individuals, families, health professionals, community partners, housing providers, voluntary organisations, and other partner agencies.
- Provide information, advice, and guidance to adults and their carers.
- Coordinate care and support to ensure individuals receive appropriate services and achieve positive outcomes.
- Support vulnerable adults while promoting informed choice, independence, and wellbeing.
- Maintain accurate, timely, and high-quality case records.
- Contribute to service development and continuous improvement initiatives.
- Support and supervise less experienced colleagues where appropriate.

Qualifications and Experience
- A recognised Social Work qualification
- Current Social Work England (SWE) registration
- Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
- Good knowledge of the Mental Capacity Act and relevant legislation
- Strong multi-agency partnership working skills
- Excellent communication, organisational, and problem-solving abilities
- A compassionate, proactive, and person-centred approach to practice
- A commitment to delivering high-quality outcomes for adults and their families
